Boston has embarked on a comprehensive health equity agenda designed to address significant disparities in life expectancy across its neighborhoods. This initiative, spearheaded by Mayor Michelle Wu and the Boston Public Health Commission (BPHC), seeks to ensure that all residents have the opportunity to live longer, healthier lives.
Understanding the Disparities
Recent data from BPHC’s Health of Boston 2023 report highlights stark differences in average life expectancy among Boston’s neighborhoods. Notably, there is a 23-year gap between residents living in parts of Back Bay and those near Nubian Square in Roxbury. Additionally, while overall life expectancy in Boston has rebounded since the COVID-19 pandemic, the racial gap has widened. In 2023, Black residents had an average life expectancy of 76 years, six years less than white residents.
Strategic Investments and Partnerships
To combat these disparities, the city has formed a multi-sector partnership involving the Boston Community Health Collaborative and the Atrius Health Equity Foundation. In July 2024, the Atrius Health Equity Foundation committed $10 million to support this agenda. An initial $5 million in grants has been allocated to community-led coalitions in Dorchester, Roxbury, and Mattapan—neighborhoods identified as having the greatest health disparities. These grants aim to build wealth and economic security, addressing the root causes of health inequities.
Community-Led Initiatives
The Boston Community Health Collaborative, facilitated by BPHC, plays a pivotal role in this effort. The Collaborative brings together city departments, public health organizations, healthcare providers, community-based organizations, and residents to develop and implement strategies that improve health outcomes and advance health equity. Their 2025-2028 Community Health Improvement Plan (CHIP) focuses on key priorities such as housing, economic mobility, access to healthy and affordable food, and access to care.

By aligning resources and fostering collaboration across sectors, Boston’s health equity agenda aims to create sustainable, positive change in the health of its residents. The city’s commitment to addressing social determinants of health through community-driven solutions underscores the importance of collective action in achieving health equity.
